Medical Insurance
There are two major public medical insurance programs in Japan: the
Workers' Health Insurance Plan which is designed for the employed and
National Health Insurance Plan (Kokumin Kenko Hoken) for all others.
The National Health Insurance Plan is managed by local cities for local
residents such as those who are self-employed. <Inquiry>

■ National
Health Insurance Plan
[Eligibility]
Except for those who are covered by other public health insurance programs
and those who are beneficiaries of a public assistance (welfare) program,
all other persons including those who have private health insurance
policies are required to enroll in the National Health Insurance Plan.
This
regulation applies to Non-Japanese residents who are registered and live
in Japan at least for 1 year.
[When to Report to the City Hall]
Report to the National Health Insurance Section within 14 days in the
following cases.
Bring your Alien Registration Card and National Health Insurance
Certificate if you have one.

(1) When you move into Matsudo City
from another city or from abroad
(2) When you withdraw from the health insurance plan provided by
your employer
(3) When a baby is born into your family
(4) When you no longer receive public assistance (welfare) benefits |

(2) When you join a health insurance plan provided by your employer
(3) When someone covered by your policy dies

City and a change concerning your family members
(2) When you have lost or damaged your National Health Insurance

*If you are to return to your native country...
Please report to the National Health Insurance Section to withdraw the
National Health Insurance before leaving Japan. Bring your National Health
Insurance Certificate and your air ticket to the National Health Insurance
section.

■ Coverage
|
When you receive medical treatment for illness or injury, the National
Health Insurance Plan covers 70% of medical expenses and you pay the
remaining 30% at the cashier's window.
However, the following treatments are not covered by National Health
Insurance:
(1) Treatments not listed for coverage such as routine medical
checkups and immunizations;
(2) Normal infant birth and abortion and sterilization (for
economic reasons);
(3) Cosmetic surgery and orthodontic work (braces). |

*Be sure to submit
your National Health Insurance Certificate to the reception window when
you go for medical treatment. Otherwise, you will be charged the full
amount of medical expenses.
By applying with required documents at a
later date, however, you will be reimbursed with a specified amount based
on the health insurance standard for the following cases:
(1)When you were unable to use your National Health Insurance Plan
for unavoidable reasons
(2)When you received massage, acupuncture, or moxibustion, or used
a corset by order of your physician
[Reimbursement for High Medical Expenses]
When the medical expenses paid by an insured person to the same medical
institution in the same month exceeds a specified amount, he/she can apply
for reimbursement of the excess amount as a high medical expense
compensation.
However, meals, differential cost of the room and other expenses which are
not covered by the National Health Insurance Plan won't apply to this
measure.

1. Childbirth Support Subsidy
(Shussan Ikuji Ichijikin) |
When a baby is born to a person covered
by the National Health Insurance Plan, 350,000 yen per live birth
will be paid. |

2. Funeral Subsidy
(Sosai Hi) |
When a covered person dies, 50,000 yen
will be paid to the person who held the funeral service. |

■ Premium
Insurance premiums are appropriated to payment of medical expenses and
subsidies for childbirth, funerals and high medical expenses for policy
holders and are the indispensable source of funds to manage the National
Health Insurance Plan. Individual premiums are calculated for the sum of
an income levy premium, a per capita levy premium, and the fixed levy
premium.
[Payment of the Premium]
(1) Go to the city hall, branch offices, bank tellers, post office or convenience
store to pay the premium.
(2) Have the premium automatically deducted from your bank account.

■ Injury
Caused by a Third Party
Medical expenses for injuries caused by a third party such as in traffic
accidents should be paid by the third party as a general rule. However,
when the third party lacks the financial capability to pay your expenses,
or when it takes a long time to recover from the injuries, you can receive
medical treatment by using National Health Insurance. In such a case, be
sure to report to National Health Insurance Section (Hoken Ka).
