When a baby is born (Live Birth Notice: Shussho Todoke)

[When to Register]

Within 14 days of birth

{Where to Report]

City hall of the place of birth or the city hall where the person who reports the birth resides

[Person who Reports]

Either the father or the mother of the baby

[Required Items]

1) Live Birth Notice (Shussho Todoke)
   *including a birth certificate completed by the attending physician
2) Mother and Child Health Handbook (Boshi Techo)
   * if you have one
3) National Health Insurance Card (Kokumin Kenko Hokensho)
   * if you have one

Registration Procedures
 (1) Live Birth Notice (within 14 days of birth) and Non-Japanese resident registration (within 60 days of birth) at the local city hall.
 (2) Apply for a passport at the respective embassy.
 (3) Apply for a visa and period of stay approval at Tokyo Regional Immigration Bureau within 30 days of birth.
 (4) Report the visa and the granted period of stay to city hall Citizen Section within 14 days of approval.

Deaths (Death Notice: Shibo Todoke)

[When to Register]

Within 7 days of learning about the death

{Where to Report]

City hall of the place of death or the city hall where the person who reports the death resides

[Person who Reports]

A family member or a person who shared the same household with the deceased

[Required Items]

1) Death Notice (Shibo Todoke)
   *including a certificate of death completed by the doctor)
2) Alien Registration Card of the deceased
3) National Health Insurance Card (Kokumin Kenko Hokensho)
   * if you have one


Marriage and Divorce Notices (Kon-in Todoke, Rikon Todoke)

{Where to Report]

If your spouse is a Japanese citizen, report your marriage or divorce to the city hall where the spouse's resident registration or family register is kept. 
If both of you are Non-Japanese citizens, report your marriage or divorce to the city hall in the area where you reside.

[Person who Reports]

For notification of marriage, both prospective newlyweds must report together.
For notification of divorce, both husband and wife must report together.

Please inquire required items from Public Service Section (Shimin Ka) in advance. 


Notes

(1) Both Birth and Death Notices must be filled out in Japanese. 
When birth and death certificates as well as the Certificate of Entitlement are prepared in a foreign language, please attach a Japanese translation for each. 
All translations must include translator's name, address and name stamp imprint (signature).

(2)The citizenship of the baby is determined by the laws of the countries of which the parents are citizens.

(3) For marriage between Non-Japanese citizens, please consult and follow instructions indicated by respective embassies or consulates.

(4) Concerning completion of marriage and divorce processes, a permission from the Legal Affairs Bureau (Homu Kyoku) may be required in some cases after receiving the applications.
